YOUR RIGHTS

Sanilac CMH is committed to protecting and respecting your rights.

When you receive mental health services, Michigan's Mental Health Code and other laws safeguard your rights. As employees of Sanilac CMH, we are responsible for protecting your rights when we provide services to you. We encourage you to ask questions about your treatment and about your rights. We encourage you to make suggestions that you feel are in your best interests.

"Your Rights" Booklet

When you use CMH services, you have a number of rights that flow from the Michigan Mental Health Code. These rights are contained in the booklet "Your Rights," which we give you when you begin receiving services. View the booklet in one of two languages below:

Filing a Formal Complaint

If you think your rights have been violated, you may file a complaint with your Recipient Rights Officer or directly with the Office of Recipient Rights in the Michigan Department of Community Health.

Complaint forms are accessible below:

Recipient Rights Complaint Form (fillable form)

Recipient Rights Complaint Form (word doc)

A complaint form is also inside the booklet "Your Rights." You can pick one up at any CMH location.

The Recipient Rights Officer will review your complaint and notify you in writing about what action will be taken. If necessary, the Rights Officer will investigate the complaint. Under certain circumstances, you have the right to appeal the outcome of the investigation. Upon your request, the Recipient Rights Officer will assist in the appeal process.

Recipient Rights Policies and Procedures

To access the Recipient Rights Policies and Procedures, utilize the Sanilac CMH Policy Manual.

Recipient Rights Advisory Committee

The Rights Advisory Council advises the Recipient Rights Officer on all issues relating to recipient rights -- the CMH's rights policies, the rights officer's activities and investigations, and rights appeals by individuals who use services.

The committee is made up of at least six members. At least one-third of the membership is made up of primary consumers or family members, and of that one-third, at least one-half are primary consumers.  The committee meets five times per year.
