ASSERTIVE COMMUNITY TREATMENT (ACT)

Assertive Community Treatment (ACT) services are generally available from 8:00 a.m. to 4:30 p.m., Monday through Friday. After hours, crisis services are available by calling the agency’s main line at 810-648-0330.

Assertive Community Treatment Services are offered at the following locations:

David Ehardt Center:  217 E. Sanilac Avenue, Sandusky, Michigan

ACT is a team-based service that includes shared service delivery responsibility, providing consistent continuity of care.

ACT is a teamwork-based approach involving experts in psychiatry, nursing, social work, substance abuse treatment, and employment services. Together, they create a personalized plan that integrates services for each individual. This eliminates the need for individuals to visit multiple locations for assistance, ensuring support is seamless and well-coordinated.

The ACT Team provides ongoing services and support tailored to individuals' evolving needs. As individuals make progress toward their recovery objectives, the ACT team gradually reduces contact but remains accessible should additional support be required.

The Assertive Community Treatment Program (ACT) is an inclusive array of community-based rehabilitative mental health services that supports individuals in their recovery journey. It is provided to adults with serious and persistent mental illnesses who have a history of high use of psychiatric hospitalizations and who require a well-coordinated and integrated package of services to avoid hospitalizations over an extended period of time.

Individuals facing severe symptoms and challenges in daily functioning, including those related to safety, self-care, employment, and housing stability, may find support through ACT services. The ACT team maintains frequent contact with individuals who require substantial assistance, providing support as needed throughout the day.
